Aging-in-Place Planning and Home Modification Assessments for Seniors

Work with a registered occupational therapist to maximize your safety and independence at home and in the community.

During a home modification assessment, an Occupational Therapist will visit you in your home to:

• Recommend equipment and strategies to make your daily activities easier
• Identify trip hazards for removal or relocation.
• Provide feedback on how to safely manage stairs, bathing, and transfers
• Provide a detailed report with photos and recommended changes

Occupational therapists are not only available to help you remain safe in your home, but to ensure all your needs from housekeeping, lawn care, snow removal, medication management, transportation, navigating friendships, family and remaining engaged with what matters most to you is addressed with aging-in-place planning!

Services are offered in-person and virtually.
